Casual Winter Pressures Support Worker

Hourly Rate: £10.00ph

Hours: Between 9am – 5pm to cover Monday- Friday (Hours to suit)

Venue: Throughout Torfaen

Hospital Discharge Service

 

We’re looking for people who are happy to work across the Hospital Discharge & Community services based at The Grange University Hospital, Neville Hall or Panteg Hospital.

The variety means you will need to be flexible, adaptable and well-organised.  This is a casual role with guaranteed hours to suit your availability.  You can accept the hours we offer you or you can say no if it’s not convenient for you.  Flexibility and reliability are important in this role.

 

Ideally, we are looking for people with experience of working in a social care environment, whether that’s at work or home.  An understanding of the challenges older people experience would be an advantage, but not essential.  What we’re looking for most is that you can show you care about people, that you can listen, and you want to make a difference.

 

Job Summary:

  • Work under the direction of the Project Co-ordinator to provide practical and emotional support to older people to enable them to remain living independently in their own home for as long as possible.
  • Supporting older people home safely from hospital discharge and settle them in after a hospital stay
  • Supporting older people in their own home, community and other settings, with a variety of tasks, based on an assessment of needs.
  • To reassure, motivate and encourage older people to maintain quality of life.

 

Responsibilities

  • Ensure the rights, dignity, welfare, confidentiality and privacy of older people.
  • Work in accordance with policy and procedures relevant to the post.
  • Provide a hospital transport service for patients leaving hospital and settle them in their own home, including taking home medication and equipment.
  • Assist with referrals for maximising income, including benefit claims and signposting to relevant services as needed.
  • Lone working with older people in their home, community and other settings.
  • Attend mandatory training, one to ones, team meetings and staff away days.
  • Flexibility will be required as determined by the needs of the service.
  • Promote Age Connect Torfaen services.
  • Any other duties relevant to the post as directed by the Project Co-ordinator
  • To comply with all Age Connects Torfaen Covid-19 regulations. Keeping up to date on changing government regulations applicable to our service users.
  • To participate in weekly Lateral Flow tests

A Full driving license and access to a car is essential for the role. There is also the requirement of ensuring you have business coverage on your car insurance.

Successful candidates are subject to an Enhanced Criminal Records check and satisfactory references.

Business Administrator

Job Title:                 Business Administrator

Report to:                Project Co-ordinator

Responsible to:       Development Co-ordinator

Job Location:          The post will be based in the Age Connects Torfaen regional office in Sebastopol but may require travel within the Torfaen area

Salary Scale Grade: £11,900 pro rata –

Hours:                     35hours

Leave:                     20 days pro rate excluding Bank Holidays

Term:                      Fixed term until July 2021

 

Main purpose of the post:

This is a newly created post funded through the Community Renewal Fund. The post includes acting as administrative assistant within a project designed to provide people aged 50+ with a selection of business start up workshops.

This is an interesting and diverse administration role and will require someone with excellent organisational skills, fully competent with Microsoft Office Tools and who delivers work with a high level of accuracy.

Key Responsibilities:

Marketing

  • Prepare publicity, newsletters, reports, press releases and update regional website with regard to project activities
  • Assist with dissemination of project outcomes/lessons learned and promote reputation of Age Connects Torfaen within the organisation and externally

Communications

  • Deal with enquiries from potential beneficiaries, tutors and partners in relation to delivery of project.
  • Assist in all aspects of project monitoring and reporting requirements of external evaluation team and UK Government
  • Comply with all Age Connects Torfaen Policies and Procedures

 

Administration ·

  • Support project team with administrative procedures relating to project registration, beneficiary enrolment, retention, achievement and progression
  • Assist in establishing and maintaining up-to-date databases (for example, updating Charity Log Database)
  • Create and maintain an up-to-date project evidence file
  • Respond to requests for information from UK Government and other relevant agencies
  • Assist in completion and submission of monitoring returns to UK Government and external evaluation team.
  • Planning and arranging internal and external meetings, video conferences and calls.
  • Assisting with correspondence, presentations and reports
  • Management of SharePoint

 

Other Responsibilities

To meet for regular supervision with Line Manager and to attend Staff Meetings

To participate in developing and maintaining good public relationships

To promote a positive image of Age Connects Torfaen to other agencies and the general public.

To work in accordance with Age Connects Torfaen policies and procedures.

This is a part time post and will require some flexibility in working patterns. The appointment is on a fixed contract until 2022.

MAIN PURPOSE OF THE JOB

 

Working throughout Torfaen, to provide befriending services for older people within the various communities of Torfaen.

 

DUTIES

 

Consistent with the policy and direction of Age Connects Torfaen.

 

SERVICE DELIVERY

 

  1. Respond to requests for information, support and befriending.

 

  1. Work closely with health, social services, care staff and other community groups that have a commitment to improving the quality of life for older people.

 

  1. Raise awareness of the Project throughout Torfaen.

 

  1. To provide a service to socially isolated people through home visits with progression to providing opportunities for social interaction and a sense of being part of a community.

 

  1. Facilitate one to one befriending sessions or telephone befriending in the home and explain the befriending service to all new members.

 

DEVELOPMENT

 

  1. To assist in the development of the Befriending Project for older people living in the various communities of Torfaen.

 

  1. Maintain the Befriending Service through “Community Hub” venues.

 

 

ADMINISTRATION

 

  1. Maintain accurate case records of older people supported through the project.

 

  1. Provide qualitative and quantitative data for monitoring and evaluation purposes as required.

 

 

OTHER DUTIES

 

Any other duties that may be appropriate and allocated from time to time following discussion.

 

On occasions, weekend working could be a possibility.
