Age Connects Torfaen has been awarded the Health Champion Award at the prestigious Mastering Diversity Awards 2025, held at the Senedd on 18th December.

The award recognises Age Connects Torfaen for the breadth and impact of services provided to unpaid carers and people living with dementia across Gwent. Age Connects was celebrated for its innovative dementia support services and for consistently thinking creatively to tailor care and support to meet the unique needs of vulnerable individuals and their families.

A core strength of Age Connects Torfaen is its co-production approach, ensuring that unpaid carers and people living with dementia have a direct voice through active participation on service user panels. This commitment to collaboration helps shape services that truly reflect the needs and preferences of those who rely on them.
