Hospital Discharge Worker

Job Title: Carers Hospital Support Worker
Responsible to: Carers Discharge Supervisor
Location: Widdershins Centre and outreach throughout Gwent.

Job Summary

The Carers Hospital Support Worker plays a vital role in supporting unpaid carers when their loved ones are being discharged from hospital. The role focuses on providing practical guidance, emotional support, and information to carers, ensuring they feel confident and equipped to manage the transition from hospital to home.

Key Responsibilities

  • Act as a first point of contact for carers at their home, providing information, reassurance, and emotional support.
  • Assist carers in understanding what to expect once their loved one returns home.
  • Carry out practical tasks within the Carers home including, light cleaning, prescription pick up, home sits and attendance to follow up appointments.
  • Support the distribution of Welcome Back packs and explain the resources available.
  • Work alongside the Carers Discharge Supervisor, hospital discharge teams, social services, and voluntary organisations to ensure carers are linked with the right support.
  • Assess carers’ immediate needs and signpost to relevant services such as financial advice, respite care, well-being programmes, and Age Connects Torfaen services.
  • Help carers access follow-up support, ensuring they are not left isolated after discharge.
  • Maintain accurate records of interactions with carers, feeding into monitoring and reporting requirements.
  • Assist with case studies and feedback collection to evidence the impact of the service.
  • Promote the Carers Hospital Hubs and ensure carers are aware of the range of resources available.

Person Specification

Essential:

  • Experience of working in a caring, health, social care, or voluntary sector role.
  • Good understanding of the challenges faced by unpaid carers.
  • Excellent communication and listening skills with a compassionate and supportive approach.
  • Ability to work as part of a multi-disciplinary team and build positive relationships.
  • Organisational skills with the ability to manage time effectively.
  • Basic record-keeping and IT skills.

Desirable:

  • Experience working directly with carers or in hospital/community settings.
  • Knowledge of local carer support services and entitlements.
  • Awareness of safeguarding policies and procedures.

Additional Information

  • The role will involve travel throughout the Gwent area.
  • Some flexibility in working hours may be required to meet the needs of carers and hospital services.

Domestic Casual Cleaner

Job Title: Domestic Cleaner

Report to: Home Services Supervisor
Responsible to:  Chief Executive
Hours: 16 hours per week over Monday- Friday
Salary: Meets National Minimum Wage
Base: Post available within Newport & Torfaen

PURPOSE OF THE POST:

Everyone likes a clean environment.  A clean home is a sign of many things,

things that reflect directly on the owner.

 

Cleaners are employed to ensure that the premises are clean and tidy.  Cleaners perform many tasks to make sure the appearance of the home is pleasing to the eye and dirt free.  Regardless of where cleaners work, they perform more or less the same type of duties.  However, to work as a cleaner, one must have a knowledge of cleaning products and equipment.  It is also important to have attention to detail, as this reflects our quality standards.

To provide an efficient and effective domestic cleaning service for older people in

Torfaen.  To ensure a professional high standard of cleaning and customer

friendly focus is delivered and maintained, to meet customers and the

organisation expectations and standards.

 

SPECIFIC RESPONSIBILITIES:

“General” and “Sparkling” cleaning service duties, for example, cleaning kitchen, bathroom surfaces, fixtures and floors, emptying bins / recycling, hovering, polishing and changing bed linen.

To support the team to carry out duties.

To undertake on the job training to develop the ability to work to the required standards

To maintain the required cleaning standards

To use correct cleaning materials to perform tasks (training provided) ensuring the following of suppliers recommended use instructions.

To use equipment within health and safety regulations and report any faults or

hazards to the client (if relevant) and Service Champion.

To adhere to COSHH regulations on consumables and the use of protective clothing and equipment.

To be responsible for both own and others who may be affected by actions in relation to Health & Safety.

To have good communication skills.

To have a flexible approach to working hours.

To have basic literacy skills to process documentation required to support the role.
