Active Connections Project Co-ordinator
Job Title: Active Connections Project Co-ordinator
Responsible to: Head of Dementia Services
Term: Fixed term until 31st January 2029
Location: Base at Widdershins Centre but project provided throughout Gwent.
Hours: 35 hours per week
Salary: £25,480 per annum
Main Purpose of the Job:
The Project Co-ordinator will lead, manage, and grow the Active Connections programme, ensuring it is co-produced with people living with dementia, their carers, and the wider community. The role will oversee day-to-day operations, staff supervision, volunteer recruitment, partnership development, and monitoring and evaluation. The postholder will ensure that all sessions are safe, dementia-friendly, inclusive, and delivered to the highest standard.
Duties and key responsibilities:
- Project Management & Delivery
- Lead the operational delivery of the Active Connections project across sports and nature programmes.
- Manage project schedules, planning, logistics, and delivery across multiple sites.
- Ensure activities are dementia-friendly, inclusive, safe, and aligned with project aims.
- Oversee improvements to outdoor and indoor spaces used in delivery.
- Coordinate resources, equipment, and materials for all sessions.
- Staff Leadership & Supervision
- Line-manage the Sports Development Officer, Forest Officer, Dementia Support Worker, and Administrator.
- Provide regular supervision, mentoring, and support to the project team.
- Ensure all staff uphold dementia-friendly practice and follow relevant policies.
- Support staff and volunteers to develop skills through training and reflective practice.
- Community Engagement & Co-Production
- Establish and lead a community-led taskforce to engage seldom-heard groups and advise on inequalities.
- Facilitate co-production workshops with people living with dementia and carers.
- Ensure lived experience shapes delivery, evaluation, and continuous improvement.
- Work closely with Memory Assessment Units, health partners, sports clubs, and third sector organisations.
- Partnership Development
- Build and maintain strong partnerships with sports clubs, leisure centres, gyms, outdoor organisations (e.g., Tidy Butt), and national sporting bodies.
- Support partners to undertake dementia-friendly audits and adapt their environments.
- Represent the project at regional meetings, networks, and community forums.
- Volunteer Recruitment & Support
- Recruit, train, and support 24 volunteers from diverse backgrounds.
- Work with the Dementia Buddy and delivery team to coordinate volunteer roles.
- Ensure volunteers receive induction, training, supervision, and recognition.
- Monitoring, Evaluation & Reporting
- Oversee all evaluation processes, including surveys, journey-tracking, case studies, and the Manchester Colour Wheel.
- Collect, analyse, and report data to demonstrate impact and outcomes.
- Work with an independent evaluator and ensure the project meets funder requirements.
- Prepare high-quality quarterly and annual reports.
- Safeguarding, Compliance & Governance
- Ensure full compliance with safeguarding, GDPR, health and safety, and organisational policies.
- Undertake risk assessments for venues, activities, and participants.
- Ensure all promotional materials and communications are inclusive and bilingual (Welsh/English).
Additional Information
- This role may involve community-based work and occasional home visits.
- A DBS check will be required.
- Travel within the local area will be necessary.

PURPOSE OF THE POST:
Everyone likes a clean environment. A clean home is a sign of many things,
things that reflect directly on the owner.
Cleaners are employed to ensure that the premises are clean and tidy. Cleaners perform many tasks to make sure the appearance of the home is pleasing to the eye and dirt free. Regardless of where cleaners work, they perform more or less the same type of duties. However, to work as a cleaner, one must have a knowledge of cleaning products and equipment. It is also important to have attention to detail, as this reflects our quality standards.
To provide an efficient and effective domestic cleaning service for older people in
Torfaen. To ensure a professional high standard of cleaning and customer
friendly focus is delivered and maintained, to meet customers and the
organisation expectations and standards.
SPECIFIC RESPONSIBILITIES:
“General” and “Sparkling” cleaning service duties, for example, cleaning kitchen, bathroom surfaces, fixtures and floors, emptying bins / recycling, hovering, polishing and changing bed linen.
To support the team to carry out duties.
To undertake on the job training to develop the ability to work to the required standards
To maintain the required cleaning standards
To use correct cleaning materials to perform tasks (training provided) ensuring the following of suppliers recommended use instructions.
To use equipment within health and safety regulations and report any faults or
hazards to the client (if relevant) and Service Champion.
To adhere to COSHH regulations on consumables and the use of protective clothing and equipment.
To be responsible for both own and others who may be affected by actions in relation to Health & Safety.
To have good communication skills.
To have a flexible approach to working hours.
To have basic literacy skills to process documentation required to support the role.
