Dementia Activities Worker

Job Title: Dementia Activities Worker
Responsible to: Dementia Services Co-ordinator
Location: Base at Widdershins Centre but home visits and supportprovided throughout Torfaen 
Salary: £12.00 per hour
Hours: 16 hours per week

Main Purpose of the Job:

To provide practical support, information and guidance to people with dementia and their carers helping to maintain their independence, improving their sense of well-being, and putting them in more control of their lives. To assist people with dementia and their carers to identify their needs and access to services.

Dimensions and limits of authority:

Responsible and accountable for the delivery of a high quality support services. Responsible for liaising with relevant statutory and voluntary organisations in the area in relation to this service.

Duties and key responsibilities:

  • To manage referrals, assisting clients in assessing dementia services.
  • To provide respite provision for carers as and when required.
  • To maintain and manage a caseload of clients who have dementia or are affected by dementia from appropriate geographical locations within Torfaen and the surrounding boroughs.
  • To support people with dementia of all ages and varying needs within our Zest for life and Merry moments groups.
  • To provide daily activities that are engaging and stimulate the needs of a person with dementia.
  • To provide 1-1 home activity sessions in the clients homes that are engaging and stimulate the needs of a person with dementia.
  • To provide information regarding Age Connects Torfaen services making the appropriate referrals as and when needed.
  • To provide an innovative person centered plan for each client and to keep this regularly update to their changing needs.
  • To maintain accurate, up to date client support plans and client contact records using the web based programme Charity Log.
  • When appropriate, to signpost clients to a choice of suitable other sources of help.
  • To develop a proactive approach in reaching people with dementia and carers who may not otherwise access our services.
  • To develop networks with partner organisations who also work in the field of dementia, mental health, disabilities, and benefits and income advice, including Primary and Secondary care, Social Care and Health, and Welfare Benefit agencies, with the aim of empowering individuals to make informed choices.
  • To assist in raising public awareness about dementia and the way it affects families and individuals, and to participate in local events such as conferences.
  • To recruit, train and support volunteers to become specialist dementia volunteers

Outreach Energy Advisor

Job Title: Outreach Energy Advisor
Report to: Development Manager
Responsible to: Chief Executive Officer
Job Location: The post will require travel throughout the borough of Caerphilly
Salary Scale Grade: £9152.00. per annum
Hours: 16 hours
Leave: 20 days pro rata excluding Bank Holidays

Main purpose of the post:

Working on our partnership project with LEAP (Local Energy Advice Partnership) Offering residents either a home visit or telephone energy assessment throughout Caerphilly. This role provides an exciting and rewarding opportunity to help the charity address fuel poverty.

This role requires a relevant Level 3 Energy Awareness qualification or willingness to complete it within 6 weeks of your start date.

Key Responsibilities:

  • Helping people with various difficulties with their energy bill.
  • Installation of simple energy saving measures including LED lightbulbs, radiators covers, chimney balloons.
  • Knowledge of energy tariffs
  • Provide day-to-day energy efficiency hints and tips to ensure residents heating systems are set up to keep warm and save money.
  • Provide energy advice.
  • Generating referrals through attending network events or meetings.

Administration Skills 

  • Updating online diary with appointments
  • Responding and creating emails
  • Assist in completion and organisation of hospitality diary and bookings.
  • Planning and arranging weekly home visits.

PERSONAL SPECIFICATION:

  • Access to a caressential for the role
  • Whilst experience of providing advice to householders and knowledge of energy efficiency would be advantageous, they are not essential as Age Connects Torfaen will provide full training to the successful candidate.

  • Excellent communication and customer care skills, in-person, telephone and written
  • Display empathy, be polite and have good listening skills
  • Able to multi-task and prioritise own work-load
  • Able to work well under pressure and have excellent time management skills

Sports Development Officer

JOB DESCRIPTION

Job Title:                 Dementia Sports Development Officer

Responsible to:       Development Manager

Hours:                     20 hours between Monday – Friday.

Location:                 Torfaen and surrounding areas

MAIN PURPOSE OF THE JOB

This varied and interesting role will support our dementia team to grow our Dementia Friendly Sports programme in the region, particularly around areas of inequality.

Reporting directly to the Development Manager and operating as part of our Dementia Services, you will provide a diverse range of sporting opportunities for people living with dementia and their Carers.

Your primary focus will be working with people with dementia with sport activities and health initiatives. You will explore, and participate in new activities which enable and encourage people living with dementia to participate in an active and healthy lifestyle, developing a greater understanding of dementia and providing the support needed to empower people with dementia to access local community provisions.

DUTIES

Consistent with the policy and direction of Age Connects Torfaen

Main Duties

  • To support, develop and deliver a weekly sporting programme throughout the year.
  • Develop good relationships with specific strategic, partners, local organisations, authorities and governing bodies and enhance funding requirements as appropriate.
  • Support and train existing dementia support workers to ensure effective running and delivery of sporting activities.
  • Maintain a good knowledge of national and local initiatives which are of benefit to Age Connects Torfaen and are in line with the organisations objectives.
  • To develop an understanding of diversity and inclusion for engaging people living with dementia who are under-represented in physical activity.
  • Deliver and evaluate a programme of training and development opportunities for members of the community related to dementia friends, activities and healthy lifestyle.
  • Effectively monitor, collect data, report and evaluate all aspects of work.
  • Work within guidelines of Age Connects Torfaen policies and procedures.
  • Carry out any other duties as required including:
  1. Attend, where appropriate, local, regional and national meetings
  2. Maintain a good knowledge of dementia and local initiatives which are of benefit to the organisation
  3. Develop good relationships with partners and local organisations
  4. Contribute to the development and delivery of strategies that encourage participation.

PURPOSE OF THE POST:

Everyone likes a clean environment.  A clean home is a sign of many things,

things that reflect directly on the owner.

 

Cleaners are employed to ensure that the premises are clean and tidy.  Cleaners perform many tasks to make sure the appearance of the home is pleasing to the eye and dirt free.  Regardless of where cleaners work, they perform more or less the same type of duties.  However, to work as a cleaner, one must have a knowledge of cleaning products and equipment.  It is also important to have attention to detail, as this reflects our quality standards.

To provide an efficient and effective domestic cleaning service for older people in

Torfaen.  To ensure a professional high standard of cleaning and customer

friendly focus is delivered and maintained, to meet customers and the

organisation expectations and standards.

 

SPECIFIC RESPONSIBILITIES:

“General” and “Sparkling” cleaning service duties, for example, cleaning kitchen, bathroom surfaces, fixtures and floors, emptying bins / recycling, hovering, polishing and changing bed linen.

To support the team to carry out duties.

To undertake on the job training to develop the ability to work to the required standards

To maintain the required cleaning standards

To use correct cleaning materials to perform tasks (training provided) ensuring the following of suppliers recommended use instructions.

To use equipment within health and safety regulations and report any faults or

hazards to the client (if relevant) and Service Champion.

To adhere to COSHH regulations on consumables and the use of protective clothing and equipment.

To be responsible for both own and others who may be affected by actions in relation to Health & Safety.

To have good communication skills.

To have a flexible approach to working hours.

To have basic literacy skills to process documentation required to support the role.
