When someone close to you is discharged from hospital, it can be an overwhelming time—especially if you’re taking on a caring role. Our Carers Post-Discharge Service is here to help you navigate this transition with the support, information, and care you need.
What We Offer:
Funded by the Gwent Regional Partnership Board, we provide practical, emotional, and personalised support to unpaid carers across GWENT who are caring for someone recently discharged from hospital for up to 6 weeks.
Our service includes:
- A Welcome Back Pack – with essential groceries, and information to help ease the pressure in the first few days
- One-to-one support from a dedicated carer support worker
- Emotional support for carer
- Help with light cleaning
- Picking up prescription
- Home sitting service
- Shopping
- Accompanying to follow up appointments
- Sign posting to outside support services
- Light meal/snack preparation
- Help accessing local services and community resources
- Advice on benefits, respite, and carers’ rights
- Emotional support and a listening ear when you need it most
Who Is It For?
This service is for unpaid carers supporting someone who has recently left hospital—whether it’s a family member, friend, or neighbour. You don’t need to be receiving formal carer support to access our help.

- Support early recovery and reduce hospital readmissions
