How to support local?

We have negotiated reduced rates with local solicitors who will draft your will in a professional and comprehensive manner. They are also able to provide advice on other legal matters such as Lasting Powers of Attorney.

Independent solicitors Rubin Lewis O’Brien have joined with Age Connects to respond to the needs of older people.  However, they are also able to offer a home visit service if required.

Hope Advocacy Project

HOPE — Helping others participate and engage

This exciting new project will be supporting people to engage, participate, gain information, have their voices heard, understand their rights, make choices, be involved, share experiences, raise awareness of advocacy, and develop skills and knowledge.

What will HOPE be delivering?

HOPE will deliver advocacy support at a community level through a range of advocacy models by recruiting and then fully training and supporting project volunteers. HOPE aims to support people at an early stage in their issues or concerns to prevent them slipping into crisis. We want to help people to get the help and support they need at the earliest point and we know that the current COVID-19 pandemic will have had a big impact on the lives of older people so they might need more support than ever to reengage with their communities and to get access to services as restrictions are lifted.

HOPE will deliver advocacy for older people (50+) and carers across Wales. HOPE staff and volunteers will also engage with communities who can guide us to make sure the service we provide is right for the needs of older people and carers in their areas. We’ll develop forums to support us to do this or we’ll work with existing ones where they exist. We don’t want to duplicate existing services but rather we want to make sure we work with and complement the existing advocacy providers and forums across Wales.

HOPE will identify and support Advocacy Ambassadors through the forums, who will promote awareness of rights and choices, and the importance, benefits and outcomes of advocacy. HOPE will promote advocacy in all its forms to show how important it is and what a difference it can make.

HOPE will establish a programme of training, workshops and events enabling advocates across Wales to further develop their skills and knowledge. Training and workshops will also be available to other professionals who would benefit from greater awareness of advocacy and how to access it.

Where and when will the project be delivering its services?

This is a Wales wide project but we’ll be working across regions with regional staff in place. These regions are:

  • Bridgend, Cardiff, Merthyr Tydfil, Rhondda Cynon Taff and the Vale of Glamorgan
  • Dyfed Powys
  • Gwent
  • North Wales
  • Swansea and Neath Port Talbot

As this is a new project, we’ll need some time to get it all up and running across some very large areas, so even within the regions we’ll have to focus our attentions on specific areas of most need in the early stages. We’ll then be rolling HOPE out across the regions. Please bear with us as we do this.

Hospital to Home Service

Many older people live alone or with an elderly partner or relative. After a stay in hospital the transition from hospital ward to home environment can be traumatic and additional support may be needed to readjust.

Age Connects Torfaen flexible Hospital to Home Service can help to get people back into their homes following a period in hospital. This help is available to people who live alone or will be alone during the day, those who live with a carer who may need support themselves or who are frail, housebound, have poor mobility and/or limited support networks in place.

Following discharge from a local hospital, the service will provide practical, emotional and liaison support to help individuals regain their independence and reduce any risk of readmission to hospital. On initial contact, an assessment will be carried out to identify immediate and short-term support needs and help to avoid duplication by identifying other agencies or services involved.

Services offered within this Hospital to Home Service may include, but not be limited to the following:

  • Transport home
  • Ensuring utilities are in working order and access to a working telephone
  • Ensuring basic provisions such as bread, milk, fruit etc. are in the home
  • If appropriate, assist with preparation of light meal/snack
  • Food delivery
  • Collecting prescriptions/medication
  • Changing of bed linen
  • Reassurance and emotional support
  • Companionship
  • Independent information, advice and welfare benefits support
  • Support to carers
  • Ensure that relevant contact details are easily accessible (after care details, and emergency details etc.)
  • Onward referral to other appropriate services and liaison support with family and partner organisations/services.

The chargeable service will operate from 09:00 – 17:00 Monday to Friday and is available to anyone living throughout Torfaen.

Local Age Connects staff will work closely with key medical, health and social care professionals to raise awareness of the project and ensure it reaches those most in need of support. The service is provided by paid staff members who have been DBS checked, and comply with current Covid 19 restrictions.

Referrals can be made through hospital staff and social workers via our Hospital to Home support worker, but we can also accept referrals from partner organisations and family/friends/support networks.

Please contact Deborah Woodrow on 01495 769264 or email Deborah.woodrow@ageconnectstorfaen.org to find out more information.

Stepping Stones Project

If you’re looking for an opportunity to top up your pension, stay active in the community, or simply learn some new skills, then our new Stepping Stones project could be for you.

Stepping Stones is a project funded through the UK Government Community Renewal Fun that connects talented, hard working people aged 50+ with a selection of business start up workshops and even the opportunity to start up your own business.

During your earlier career, you may have struggled to find a work-life balance, setting aside passions and interests in favour of job stability, earnings, and progression. Or perhaps you’ve already retired and miss the sense of purpose that comes from being at work. Whatever the reason, more and more people in their 50s, 60s, and beyond are finding fulfilment from starting a new part-time career.

From January 2022, Stepping Stones will offer 12 week programmes, of once weekly workshops, taught by experts in their field. The programmes on offer are;

  • Ceramics
  • Jewellery Making
  • The Repair Shop (upcycling and DIY)
  • Growing and managing your own allotment
  • Cake Making
  • Home made foods

If you’ve got a hobby or skill that has the potential to earn you some money, then why not take the leap and give it a go?

For example, if you’re skilled in arts and crafts, you could hand-craft items to sell at marketplaces or online on websites such as Etsy. This could allow you to practise and develop your skills whilst earning. Becoming a small business owner is not a fast track to a large sum of money, but it’s a chance to take control of your professional life and reignite old passions.

The Stepping Stones project will provide 16 selected people with further support for individuals who want to start their own business. Working with our partners Communities for Work+, we will provide business support workshops and on completion of a 12 week course, provide you with a business set up support grant of £2500 (with set parameters that the money is to be used for items including uniform, website, insurance, registrations fees etc).

Finding a new role that suits you may involve a bit of trial and error. This stage of life can be a great time to try something different and take on new and exciting challenges. If you don’t find the right role straight away, don’t give up – there’s something out there for everyone. Our Stepping Stones project also offers individuals the opportunity to enhance their skills by taking part in a 12 week workshop within one of our Age Connects trades. These areas include:

  • Health & Social Care
  • Health & Beauty
  • Business and administration
  • Catering and hospitality
  • Caretaking

For further information, please complete the referral form below and we will be in touch. Alternatively, you can give us a call on 01495 769264.
