We are thrilled to share the fantastic news that Age Connects Torfaen has successfully secured funding from The National Lottery Community Fund to deliver an exciting new three-year programme called Active Connections.

This significant investment will allow us to build on our existing work and support older people and those living with cognitive impairment to stay active, connected, and engaged within their communities across Torfaen and the wider Gwent area.

About Active Connections

Active Connections is a community-led project designed to improve physical health, mental wellbeing, and social connection through inclusive, accessible activities. The project will focus on helping people remain active in ways that are meaningful, enjoyable, and tailored to individual needs.

Over the next three years, Active Connections will deliver a range of activities that promote movement, confidence, and connection, while reducing isolation and supporting independence. The project has been shaped directly by feedback from people with lived experience, carers, local partners, and community organisations.

What the project will deliver

The funding will enable us to offer a combination of gentle physical activity and outdoor and nature-based sessions, delivered in safe, welcoming, and dementia-friendly environments. Activities will be available in group settings, one-to-one support, and joint sessions where family members and carers can take part together.

We will work closely with local sports clubs, community venues, and partner organisations to ensure activities are inclusive and adaptable, while also supporting those venues to become more dementia-friendly.

A truly community-driven approach

Active Connections has been developed following extensive consultation with people living with dementia, carers, health professionals, voluntary organisations, and underrepresented groups. This has ensured the project responds to real needs, addresses gaps in local provision, and reflects the strengths and diversity of our communities.

The project will be delivered bilingually in English and Welsh, ensuring accessibility and inclusion for all participants.

Looking ahead

This funding marks an exciting step forward for Age Connects Torfaen and for the communities we support. Over the next three years, Active Connections will help people stay active, build friendships, enjoy the outdoors, and maintain a strong sense of purpose and wellbeing.
